JOB REQUIREMENTS
Education
Licensed, with at least a Bachelor’s degree in Social Work from an accredited program. Experience in geriatrics and / or older adult services preferred.
Experience
Three years of experience in a health setting or supervisory / consultative responsibility in a social agency with acceptable standards required. One to two years of experience providing social work case management services to a frail or elderly population preferred.
Physical
Normal LTC environment. Possible exposure to biological hazards or unpleasant odors. Close eye work. Hearing of normal and soft tones. Distinguish smells. Distinguish temperatures by touch and proximity. Lifting up to 100 lbs. Carrying up to 40 lbs. Pushing / pulling up to 350 lbs. Frequent sitting, standing, walking, bending, stooping, and reaching.

JOB SUMMARY
The Social Worker will carry out, in collaboration with appropriate allied health professionals, the provision of services to individuals, groups, and community as required. They will be responsible for consultation with other agency personnel and for direct services to residents. The Social Worker must interact with all other housing departments, promote good public relations for the division, and the medical center. Must be able to work flexible hours. Must possess a valid driver’s license. Must be able to relate to residents, family members, and staff in a courteous and diplomatic manner under all circumstances. Highly dedicated, passionate, creative, and focused.
