Salary : $70,000 - $85,000 annually

Responsibilities :

Monitor and accurately record patients' vital signs

Ensure adherence to medication schedules and provide guidance to patients

Observe, document, and report patient progress throughout treatment

Communicate regularly and effectively with patients' families regarding health and progress

Conduct follow-up visits to support ongoing home health care needs

Qualifications :

Active LVN license in Oregon

Home health experience preferred

Valid driver's license with a clean driving record

Compassionate, detail-oriented, and excellent communication skills
