Provides therapeutic intervention and social work services to patients and their families to enhance comprehensive, integrated, and uninterrupted care in the hospital and ensure continuity of care in the community. ??? Assesses patients??? psychosocial situations in relation to medical needs and plans appropriate interventions and treatment goals to improve patient / family functioning. ??? Interviews patients, family members, and / or significant others identified through case finding or referrals to assess needs for social work and / or interventions related to social determinants of health. ??? Participates in and provides input during multidisciplinary rounds regarding psycho / social context. ??? Functions as a resource and advocate on behalf of the patient and family for discussions involving advanced care planning. ??? Provides psychosocial support to patients and families during adjustment to illness and life changes. ??? Collaborates with the health care team regarding in-hospital needs and post-hospital care. ??? Coordinates psychosocial discharge planning, including long-term care, acute / sub-acute rehab, SNF, LTAC, dialysis, hospice, and homelessness resources. ??? Documents assessments, interventions, family contacts, and discharge planning according to department policy and regulatory requirements. ??? Develops and maintains relationships with community organizations and government services to connect patients with benefits, housing, food support, and financial resources. ??? Graduate of an accredited school of Social Work (MSW). ??? Previous medical hospital social work strongly preferred. ??? Excellent communication, negotiation, and conflict resolution skills. ??? Knowledge of trends and advances in clinical practice, healthcare informatics, and relevant regulatory standards. ??? Clinical performance improvement and rapid cycle change expertise. ??? Ability to meet physical demands of the role (standing, walking, handling / lifting, visual / hearing acuity). Reasonable accommodations may be made.
