Key Responsibilities

Deliver telephonic care management for patients in Value-Based Care Programs

Develop patient-centered care plans and educate patients and caregivers

Communicate patient health updates and coordinate care with providers and support teams

Required Qualifications, Training, and Education

Active, unrestricted compact LPN license

2+ years of nursing experience in various care settings

Knowledge of pathophysiology and treatment protocols for common health diagnoses

Proficient computer skills and ability to work in a remote environment

Experience with clinical data to guide decision-making
