Position Summary
Our Patient Navigators play a vital role in making sure no patient feels lost in the process. As a key member of our care team, you'll be the personal guide for patients with complex or ongoing treatment plans, making sure they feel informed, prepared, and supported at every step. You’ll combine clinical knowledge, empathy, and communication skills to coordinate follow-up care, educate patients, and remove barriers that could delay progress. Job Relationships :
Reports to the Navigation DirectorCollaborates with Physicians, APPs, Scheduling Teams, Front End Teams, and Clinical Support TeamsSupervises : NoneQualifications
Required : Active Medical Assistant (MA) certification and at least 2 years of experience in a clinical or specialty care settingPreferred : LVN or RN licensurePreferred : Previous experience in urology or other longitudinal care specialtiesPreferred : Strong knowledge of urologic conditions, procedures, and patient educationProficiency in electronic medical records (EMR) and patient tracking softwareExcellent interpersonal, organizational, and time management skillsEssential Job Duties
Serve as the primary point of contact for patients with multi-step or complex care plansProvide clear, compassionate guidance on upcoming appointments, procedures, and treatment milestonesConduct timely outreach to patients at designated intervals, reinforcing adherence and addressing questionsIdentify and document barriers to care, and escalate issues to appropriate team members when neededTrack patient progress along clinical pathways and ensure timely coordination of services (labs, imaging, referrals, procedures)Collaborate closely with providers, front office staff, and clinical teams to ensure care moves forward as orderedAccurately document all patient communication, care plan updates, and task completion in appropriate systemsReinforce provider instructions and offer education tailored to each patient’s treatment planMonitor patient records to flag delays or missed steps, proactively resolving any gapsParticipate in ongoing discussions to refine pathway protocols and support workflow improvementPromote a positive patient experience through timely, respectful, and professional communicationMaintain strict confidentiality and comply with all HIPAA and clinic policiesPerform additional navigation or support tasks as assignedPhysical Demands and Work Environment :
