Physical Therapist – Long-Term Care / SNF - Travel Position

Location : Jackson, TN

Job Type : Travel Contract

Duration : 13 weeks

Shift : 5x8s Days

Pay : $2,165 / week

Job Description

We are seeking a compassionate Physical Therapist to join a skilled nursing and long-term care facility in Jackson, TN, on a travel contract. The Physical Therapist will evaluate residents, create individualized treatment plans, and deliver therapy services aimed at improving mobility, restoring function, and promoting independence. The ideal candidate is patient-centered, detail-oriented, and experienced in geriatric rehabilitation.

Key Responsibilities

  • Conduct comprehensive evaluations to assess residents’ functional abilities and therapy needs.
  • Develop and implement individualized treatment plans in collaboration with physicians and interdisciplinary staff.
  • Provide therapeutic interventions including strengthening, gait training, balance activities, and mobility support.
  • Educate residents, families, and caregivers on therapy goals, safety practices, and use of assistive equipment.
  • Document evaluations, treatment sessions, and progress notes accurately and timely.
  • Collaborate with nursing and therapy staff to ensure coordinated, patient-centered care.
  • Promote safety, independence, and improved quality of life through evidence-based therapy practices.

Qualifications

  • Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.
  • Current and valid Physical Therapist license in Tennessee (or compact license).
  • Minimum of 1 year of recent PT experience, preferably in skilled nursing or long-term care.
  • Current certification in Basic Life Support.
  • COVID-19 vaccine and recent tuberculosis test.
  • Strong communication, assessment, and organizational skills.
  • Compassionate approach with experience supporting geriatric populations.
