Join a mission-driven community health center in Jackson Heights, NY that delivers high-quality, compassionate, and culturally responsive care to patients of all backgrounds.

We proudly serve a diverse patient population, including LGBTQ+ individuals, people living with HIV, and refugees and immigrants in a primary care setting.

What You’ll Do :

  • Provide case management to Primary Care patients; including comprehensive assessment, reassessment, and individualized care plan
  • Provide crisis intervention, stabilization, and brief counseling
  • Coordinate and link patients with additional services
  • Participate in community events
  • Other relevant duties as assigned

What We’re Looking For :

  • Master's degree in mental health counselling or social work required.
  • Two (2) years work experience working with underserved individuals, including children and family services, individuals with chronic mental illness, substance use disorder, LGBTQ+, individuals living with HIV / AIDS, refugees and / or immigrants.
  • Must have an active LMSW or LMHC
  • Fluency in Spanish or one of Asian languages a plus.
  • Ability to work flexible hours including some evenings and weekends required
