What You’ll Do :
- Provide case management to Primary Care patients; including comprehensive assessment, reassessment, and individualized care plan
- Provide crisis intervention, stabilization, and brief counseling
- Coordinate and link patients with additional services
- Participate in community events
- Other relevant duties as assigned
What We’re Looking For :
Master's degree in mental health counselling or social work required.Two (2) years work experience working with underserved individuals, including children and family services, individuals with chronic mental illness, substance use disorder, LGBTQ+, individuals living with HIV / AIDS, refugees and / or immigrants.Must have an active LMSW or LMHCFluency in Spanish or one of Asian languages a plus.Ability to work flexible hours including some evenings and weekends requiredWhy You’ll Love Working Here :
