Pediatric Registered Nurse - Home Care

The Caswood Group has 2 openings for for RNs with Pediatric and Home Care experience for a 13 week travel contract in Monroe County.

Opening 1 :

Monday - Friday : 8am - 4 : 30pm EST

Opening 2 :

32hrs / week Friday - Sunday 8am -4 : 30pm

BOTH POSITIONS will train for first two weeks of contract M-F 8am - 4 : 30pm

Responsibilities of the Pediatric Home Care RN :

• Develop care plans appropriate to the patients / families health care needs in conjunction with patient, family, physician, clinical operations team and third party payers including obtaining medical orders and approval of medical services.
• Completes the OASIS Assessment as needed to accurately reflect the clinical, functional and social needs of the patient.
• Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established time frames.
• Refers to appropriate Health Care Systems, URMHC and community agencies / resources.
• Communicates to the health care team members the outcome of the assessment / reassessment (if applicable) visit to ensure appropriate follow up occurs based on the

needs of the patient / family’s plan of care.

QUALIFICATONS :

Education & Experience - Applicants must possess the following qualifications :

One year of experience as a clinical pediatric nurse strongly preferred, home care experience preferred.
