Job Summary

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in various settings, including hospitals, long-term care facilities, and outpatient clinics. The LPN will work under the supervision of registered nurses and physicians, ensuring that patients receive the best possible care tailored to their individual needs.

Duties

• Administer medications and perform treatments as prescribed by healthcare providers.
• Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
• Assist in the development and implementation of patient care plans.
• Provide personal care and support to patients, including assistance with daily living activities.
• Document patient information accurately in electronic health records (EHR) systems.
• Collaborate with interdisciplinary teams to coordinate patient care effectively.
• Educate patients and their families about health management and disease prevention.
• Maintain a clean and safe environment for patients, adhering to infection control protocols.
• Participate in quality improvement initiatives to enhance patient care services.
