Requirements :

Board Certified - REQUIRED if Board Eligible (ABIM ONLY)

Active WA license or IMLC (LOQ in hand / must upload during name clears) - REQUIRED

Completed Endocrinology Fellowship - REQUIRED

Malpractice - Clean Preferred

Availability at time of name clear - REQUIRED

ACLS / BLS / DEA - REQUIRED
