Position Overview : The LPN plans, directs and coordinates medically approved programs for patients in hospitals, nursing homes, substance abuse facilities, or other institutions. May assess a patient condition and recommend a recreational activity. May assist the Registered Nurse with the nursing process with individual patients and / or groups of patients to meet their physiological, psychological, safety, and general health needs.
Specific Responsibilities :
Charge LPN responsibilities (if applicable) :
1.Make the assignment sheets for the shift and communicate it to staff (including staff assignment and breaks)
2.In collaboration with the program manager / leadership team is responsible for community management ensuring a safe and efficient shift flow
3.In the absence of DON and / or Program Manager, ensures completion off assignments, offers mentoring and provides feedback as needed
4.Accepts call out notifications from staff and follows all necessary actions / steps to find coverage and ensure unit is adequately staffed for the shift
5.Acts as the centralized person to report on a patient's progress and / or special circumstances and concerns to the doctor or the AOC as needed
6.Serves as the main contact for employees / emergencies in the facility
7.Report personnel and administrative issues to DON / leadership
8.In collaboration with DON / leadership, informs staff of changes in protocols and procedures
9.Has the ability to prioritize and adapt during emergencies

The employee must occasionally lift and / or move objects up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision and the ability to adjust focus.
Travel : Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
