Join our team as a Care Manager / Case Management Registered Nurse, where you will coordinate personalized nursing care plans and continuously assess patient conditions to ensure high-quality care tailored to individual needs. These principles guide our professional conduct and interactions with patients and colleagues alike.\n\nAs a Care Manager, you will manage individualized, goal-directed nursing care through the nursing process and the principles of primary nursing. This position is in accordance with departmental and hospital policies and procedures.\n\nResponsibilities\n

  • Continuously assess the patient's physical and psychological condition throughout their care, documenting significant information and sharing it with the appropriate disciplines.\n

  • EDUCATION : Graduate of an accredited school of nursing required.\n
  • EXPERIENCE : Previous experience in geriatric psychiatric nursing preferred; prior hospital experience is beneficial.\n
  • LICENSURE : Current Registered Nurse license required; Basic Life Support (BLS) certification needed; Crisis Prevention Intervention (CPI) training to be completed within 1 year of employment.\n
