The Role
As a Behavioral Care Manager (BCM), you’ll work side-by-side (virtually) with primary care teams to identify, treat, and follow patients who need behavioral health support — all within the Collaborative Care Model.
This isn’t traditional therapy. You’ll use short-term, evidence-based interventions, track measurable progress, and help guide treatment adjustments in real-time — so patients get the right care, at the right moment.

What You’ll Do
Work closely with primary care providers to coordinate behavioral health care for their patientsScreen and assess for depression, anxiety, substance use, and other conditions using tools like PHQ-9, GAD-7, CSSRSMonitor patients for progress, with a goal of 50% or 10-point reduction in symptomsSupport PCP-led medication management by tracking adherence, side effects, and effectivenessUse a patient registry to follow up, re-engage, and adjust care plansFacilitate changes when patients aren’t improving — from medication tweaks to therapy referralsDocument progress and recommendations in the EHR for easy team accessComplete relapse prevention plans for patients in remissionEducate patients and families about conditions and treatment optionsJoin regular caseload consultations with a psychiatric consultant for support and guidanceWhat We Offer
Pay : $45–$75 / hour (based on experience) + up to 10% biannual performance bonusPaid for ALL Your Time : We pay for every minute you work with us — including and not limited to direct patient care, documentation, training, meetings and professional development. Master’s degree + active license in New York (LMSW, LCSW, LMHC, LMFT, or equivalent) — associate or independent licensure welcomeAt least 2 years of experience as a licensed mental health professionalExperience with either pediatric or geriatric populaiton (preferred)Familiarity with evidence-based approaches like PST, MI, CBT, Behavioral ActivationComfort with EMRs and tech toolsPrior remote work experienceKnowledge of the Collaborative Care Model (preferred)This Role Isn’t for You If
