InterCommunity, Inc is a FQHC LA (Federally Qualified Health Center Look Alike). Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services. Our community Health Care Centers are located at 281 Main St. East Hartford, 40 Coventry St. Hartford and 828 Sullivan Ave., South Windsor.

InterCommunity’s Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area. We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.

Summary :

The RESET Support Specialist provides skill building, engagement, care coordination as part of the Young Adult Services Team. Services include both milieu supervision as well as community-based services to program participants in a transitional home environment.

Essential Duties & Responsibilities :

Assist in linking each client between the ages of 18-25 years old to the necessary clinical, medical, social, educational, rehabilitative, vocational and / or other services within the team or outside providers if necessary.

Supervision of participants in the milieu and during on-site and scheduled off-site activities; may conduct room safety checks.

Collaborate within the team and the clients to identify required services by assessing the overall needs of each client.

Support client recovery by participating in individual treatment sessions within the YAS Team or outside providers if necessary.

Completes all required documentation related to treatment in accordance with established procedures including but not to limited to court letters, gathering medical records, copying, faxing, scanning, collateral contact and appointment scheduling.

Proficient knowledge with computer and technology skills (EMR System, Typing, Emailing, use of Smart Phone & Microsoft office).

Ability to create and facilitate groups based on client identified needs (both onsite and community based).

Provides assistance during the assessment of individuals in crisis and intoxicated clients by helping to determine the need for crisis involvement or detox and facilitate the admission to detox facility.

Provides education, support and consultation to clients and the individual’s families or their support system which is directed exclusively to the well being and benefit of the individual, if needed.

Performs related duties as required including housekeeping and transportation of clients.

In the event of staff shortage, may be required to work past scheduled hours until a relief can be found.

All agency staff are required to attend all mandatory department / agency meetings and trainings

All Residential Support Staff Employees MUST show proof of CPR Certification within 60 days of official start date.

Schedule :

Tuesday-Saturday 7 : 00am-3 : 30pm, plus shift differentials

Wage Range :

$21.25 - $25.00 Hourly

Requirements :

Education & / Or Experience :

A bachelor’s degree or an associate degree in health or behavioral health. Recovery Support Specialist (RSS) Certification or equivalent experience in a community health center or behavioral health setting may be considered. Experience working in a setting attending to the needs of those in recovery from mental illness and substance use / addiction.
