About The Position

Must have :

MIDAS / Meditech Experience, PASRR, Navihealth

MSW or LCSW

List Typical Procedures Performed on Unit(s) :

Discharge planning, MDR, Referrals, Working with Family and Patients

Number of Beds : 344 beds at acute care hospital (excluding BH and acute rehab)

Patient Ratios : SW if pair with Matrix of another RN will work Complex case of 8 or 9. If they are just covering DC planning then will have 1 : 25 or up to 30

Documentation System : MIDAS / Meditech, Nate, Navihealth, Care Team Assignment

Call Requirements : All work every third weekend

Floating policy : All Social workers float-except trauma.

Patient Experience requirements : Knowledge of Hospital DC planning / execution and knowledge of MDR or IDT
