We are seeking a knowledgeable and dedicated MDS Director to join our interdisciplinary team of skilled health care professionals at our skilled nursing facility in Flushing, Queens!
Job responsibilities include
- Completing accurate assessments, MDS & care plans as assigned.
- Monitors MDS and care planning documentation for all residents; ensures documentation is present in the medical record to support MDS coding.
- Initiating care plans and supporting activities as assigned.
- Maintaining & updating all care plans and assessments as required.
- Monitoring & auditing clinical records, ensuring accuracy & timeliness.
- Protecting the confidentiality of Resident & Facility information at all times.
