A company is looking for a remote pharmacist licensed in Pennsylvania, New York, and / or South Carolina.
Key Responsibilities
Conduct thorough medication reviews and collaborate with healthcare providers to develop management plans for pediatric patients
Provide remote consultations to caregivers and offer recommendations for medication adjustments and adherence strategies
Educate pediatric patients and caregivers on medication use and participate in interdisciplinary healthcare team meetings
Qualifications
Doctor of Pharmacy (PharmD) from an accredited program; active pharmacy license required
Board certification in Pediatric Pharmacy (BCPPS) preferred
Minimum 4 years of pediatric pharmacy experience with strong clinical knowledge
Proficient in telehealth platforms and EHR systems
Proven ability to work independently and manage caseloads effectively
