• Resume and license verification attached.
• Covid test 72 hours prior to start date, with results issued 48 hours prior.

Description :

Ensure prescreen form is filled out and attached upon submission.

Resume and license verification must also be attached.

If these items are not completed and attached, the candidate will be declined.

New Graduates are welcome to apply.

Experience in Skilled Nursing Facilities (SNF) is preferred.

Covid testing must be conducted 72 hours prior to the start date, with results issued 48 hours prior to the start date.

Candidates should review the credentialing checklist prior to submission.

Credentialing requirements include state license verification, resume, and three references.

A county criminal background check (7 years for all counties lived and worked) is required.

A federal criminal background check (7 years or more) is needed.

A multi-state criminal search (7 years or more) is necessary.

A nationwide sex offender search (7 years or more) is required.

HHS / OIG search and GSA search must be completed.

A social security number trace is needed.

A 1 step PPD or Quantiferon or proof of positive PPD and chest X-ray documentation is required.

A SAM search must be completed.

The Department of Justice Check (DOJC) is required, to be re-ran every four years with BID form.

Position urgency is normal.

Bilingual skills are not required.

No holiday coverage is necessary.
