The OT will assess, plan, and implement rehabilitative interventions to improve patients’ functional independence, mobility, and quality of life. This role requires strong clinical judgment, documentation accuracy, and effective interdisciplinary collaboration. Responsibilities Evaluate and develop individualized treatment plans for patients with cognitive and / or ADL (Activities of Daily Living) impairments in accordance with clinical and regulatory standards. Utilize standardized assessments and evidence-based interventions to guide therapy plans. Administer treatments, including ADL retraining, therapeutic exercises, modalities, and use of assistive devices. Monitor and document patient progress; adjust care plans as needed to achieve optimal outcomes. Provide education and counseling to patients, caregivers, and families to support continuity of care. Communicate effectively with supervisors, interdisciplinary team members, and facility staff regarding treatment goals and patient progress. Complete accurate documentation and treatment minutes per regulatory and facility requirements using the electronic documentation system. Prepare discharge summaries and recommendations for continued care as appropriate. Participate in required care conferences, utilization reviews, and facility meetings. Supervise and support COTAs, rehab aides, and therapy students as applicable. Maintain adherence to NBCOT / AOTA standards, state licensure requirements, and professional ethics. Submission Requirements Experience : Minimum 1 year of OT experience required (LTC or Rehab experience required). New Grads : Accepted if Level II fieldwork experience completed (Level I observation only not sufficient). Licensure : Active Missouri OT license – required.
