Job Description

American Traveler is hiring an experienced RN for a Home Health position with hospice case management duties and a requirement for a minimum of two years acute care experience.

Responsibilities

  • Work in a Home Health and Hospice care setting
  • Responsible for a patient caseload of 12 to 15 hospice families
  • Schedule is Monday through Friday, 5x8 hour day shifts from 8 : 00 to 16 : 30
  • Uses Epic and Remote Client EMR platforms
  • Perform comprehensive assessments, develop individualized care plans, and provide skilled nursing care to home health and hospice patients
  • Manage pain and symptoms, wound care, admissions, discharge, and recertifications
  • Coordinate care and collaborate closely with the interdisciplinary team
  • Accurate and timely clinical documentation is required
  • Assist with family bereavement services following patient death
  • High level of team coordination is expected
  • Local candidates are not accepted for this position
  • First-time travelers with previous hospice experience are welcome to apply

  • Active Oregon RN license required
  • Minimum of 2 years acute care RN experience required
  • Prior hospice experience required for first-time travelers
