About The Position

Job Title : Hospice Case Manager RN

Floating within the facility or to affiliated locations is required.

Experience in home health or hospice is essential.

Description :

The Hospice Case Manager RN is responsible for providing comprehensive nursing care to patients in a home setting.

The role involves assessing patient needs, developing care plans, and coordinating services with the healthcare team.

Candidates must have experience in pain assessment, management of intravenous therapies, and patient / family education.

Strong skills in communication and collaboration with interdisciplinary teams are essential.

The position involves working with diverse patient age groups, including infants, children, adults, and elderly patients.

Candidates should be familiar with managing symptoms such as nausea, vomiting, and pain, utilizing various pain scales.

The role requires effective goal setting with patients and their families while addressing advanced directives and end-of-life care.

As the Hospice Case Manager RN, the candidate will provide emotional support and counseling to patients and their families.

Charting and documentation in the specified electronic health record system is part of the responsibilities.
