Job Summary :

Primary functions are to carry out clinical examination of patients in their care, perform diagnosis based on their assessment of the patient, and coordinate care with the interdisciplinary group.

Responsibilities :

• Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, performs face-to-face assessments, and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team.
• Provides medical care, palliates, and manages terminal illness in compliance with the state Nurse Practice Act.
• Completes history and physical examinations by evaluating the patient’s medical condition.
• Manages health conditions and terminal illnesses.
• Completes the hospice face-to-face visit requirements.
• Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification.

Environmental and Working Conditions :

Works in patients' homes in various conditions, possible exposure to blood and bodily fluids and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call.

Physical and Mental Effort :

Prolonged standing and walking are required, with the ability to lift up to 50 lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient / family individualized psycho-social needs.
