Temah Family practice is looking for a Family Nurse Practitioner or an Adult Nurse Practitioner who is willing to work part-time or full-time.

Job Responsibilities

Diagnosing and treating acute illnesses, infections, and injuries such as medical illness.

Management and treatment of common chronic conditions such as medical illness

Telemedicine / telephonic consultations

Chronic Care Management of medical illnesses

Diagnosing and Treating substance use disorder including opioid use disorder

Medication management including suboxone and monthly vivitrol

Ordering, performing, and interpreting diagnostic studies.

Medication management and reconciliation

Ordering, performing, and interpreting diagnostic studies

Collaborating with physicians, mental health team, and other health professionals

Referral of patients to specialist as indicated including mental health

Referral of patients to physical, occupation, and speech therapy (worked with Baltimore Ortho)

Provides health education patients and families and connecting them to necessary resources

Coordinating care to contribute to reduction in re-hospitalization

Patient Advocacy

Administer age- appropriate immunization

Perform other duties as assigned
