Job Description

Job Title : Registered Nurse

Experience : Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience

License : Must have a license in hand; no pending licenses accepted

Certifications : CPR and BLS required

Must-Have :

Proficient in OASIS and HomeCare HomeBase EMR, used within the last 6 months
Reliable transportation, current driver's license, and agency-required liability insurance

Job Description :

Provides, coordinates, and directs the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process

Responsible to the Clinical Manager

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses

Completes home visits and attends periodic office meetings

Manages an average of 5-8 patients per day, aiming for a productivity goal of 30 points per week

Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.

Covers a territory with a travel requirement of up to 75 miles from the home clinic; mileage is reimbursed

Documentation must be completed within 24 hours of the appointment

Participates in on-call rotations, typically every 3-4 weeks

Must be willing to float to other nearby clinics if patient census is low

Proficient in Pointcare, HomeCare HomeBase, and Forcura EMR systems
