Job Summary : \n\nSN Homehealthcare is seeking an experienced and compassionate Registered Nurse (RN) to provide comprehensive, patient-centered care in the comfort of patients' homes. As an RN, you will assess, plan, and deliver individualized care to a diverse patient population while coordinating with interdisciplinary team members. The ideal candidate is self-motivated, organized, and committed to improving the health and well-being of patients.\n\nKey Responsibilities : \n\nConduct initial and ongoing patient assessments to determine health status and care needs.\n\nDevelop, implement, and regularly update individualized care plans in collaboration with the patient, their family, and the care team.\n\nProvide skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.\n\nEducate patients and their families on health conditions, treatment plans, medication use, and preventive care strategies.\n\nSupervise and mentor Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) to ensure quality care delivery.\n\nCoordinate with physicians, therapists, social workers, and other healthcare professionals to ensure seamless care transitions and effective treatment outcomes.\n\nMonitor patient progress, document observations, and communicate any changes in condition to the care team promptly.\n\nEnsure compliance with agency policies, state and federal regulations, and accrediting standards.\n\nUtilize electronic medical records (EMR) to maintain accurate and timely documentation.\n\nParticipate in case conferences, quality improvement initiatives, and ongoing training\n\nJob Qualifications\n\nQualifications : \n\nCurrent and valid Registered Nurse (RN) license in (State).\n\nBachelor's Degree in Nursing (BSN) preferred but not required.\n\nMinimum of 2 years of clinical nursing experience, preferably in home health, acute care, or community health.\n\nCurrent CPR and First Aid certification.\n\nProficiency in clinical skills, including assessment, care planning, and critical thinking.\n\nStrong interpersonal and communication skills for patient and family education.\n\nAbility to work independently, manage a flexible schedule, and make decisions in home-based settings.\n\nReliable transportation and a valid driver's license.\n\nExperience with electronic medical records (EMR) systems is a plus.\n\nJob Types : Part-time, Per diem\n\nPay : $ $50.00 per day\n\nAbility to Commute : \nWellington, FL Required)\n\nAbility to Relocate : \nWellington, FL 33414 : Relocate before starting work (Required)\n\nWork Location : In person
