About The Position

  • Evaluate residents mobility, functional limitations, and rehabilitation needs through initial and ongoing assessments.
  • Develop and implement individualized, person-centered treatment plans to improve physical function, reduce pain, and prevent disability.
  • Deliver direct skilled therapy interventions, including therapeutic exercises, gait and balance training, and application of physical modalities.
  • Monitor residents progress and adjust care plans as needed to ensure optimal outcomes.
  • Collaborate with interdisciplinary care team and participate in care planning, including discharge planning.
  • Educate residents, families, and caregivers on home exercise programs and safety to encourage continued independence.
  • Supervise and direct Physical Therapist Assistants (PTAs) and therapy aides as per licensure requirements.
  • Maintain accurate, timely documentation of treatment sessions, progress notes, and billing records.
  • Ensure compliance with state, federal, and facility regulations and standards for physical therapy.
  • Contribute to quality assurance and performance improvement initiatives within the rehab team.
  • Work in a skilled nursing / long-term care facility, requiring physical activity such as standing, bending, lifting, and assisting residents.
  • Operate in a fast-paced, team-oriented rehabilitation environment.
