For a limited time MaineHealth is offering a $10,000 Sign on bonus for all eligible Registered Nurses with up to 2 years of RN experience and $20,000 for Registered Nurses with greater than 2 years of RN work experience!
This role is an integral member of the palliative care team serving patients in Cumberland and York counties in-person in their home as well as by phone and telehealth. This role may also provide care to patients and families in the outpatient palliative care clinic in Scarborough.
The Palliative Care Registered Nurse is an integral member of the Palliative Care Interdisciplinary Team which delivers patient- and family-centered care to individuals living with serious illness. The goal of Palliative Care is to maximize quality of life and align the individual's medical care with what matters most to them.
Key responsibilities of the Palliative Care RN include coordinating care, conducting comprehensive symptom assessments, promoting understanding of disease processes and treatment options, facilitating family meetings and goal-setting discussions, and engaging in effective interdisciplinary team collaboration.
This is a full-time, 40-hour position during the Monday-Friday work week. Some flexible scheduling me be possible.
Summary :
The Registered Nurse - Home Care - MMCP role provides patient-family centered quality care through assessment, outcome identification, planning, implementation, documentation and evaluation on an outpatient and / or community basis.
