JOB SUMMARY
The Medical Assistant (MA) supports Lightpath Health's (LPH) programs by providing proactive individualized coaching and resources linking through in person and virtual methods with an end goal of affecting patient healthy living and behavioral change. They will assemble and be the content expert on a list of area community resources to improve the health of the managed population. The Medical Assistant will foster an environment of individual responsibility through patient education and support. Reporting to and working closely with Management, the Clinical MA is tasked with achieving system goals of improving health and clinical outcomes for patients by using timely and appropriate coordination of quality healthcare services to meet an individual's specific health needs to promote positive outcomes.
R ole and Responsibilities
- Support in person and virtual outpatient care and the Provider's needs by performing medical assisting duties, including but not limited to greeting and rooming patients, assisting patients with virtual connections, obtaining patient vitals, ROS, and history, reviewing plans of care or instructions with patient, conducting minor tests, and / or administering vaccinations.
- Administer health assessments to patients through in person or virtual methods.
- Provide needed follow up with the managed population related to chronic condition management, educational health information, appointment requests, and monitors patient communication pathways as needed.
- Work closely with multi-disciplinary Care Team members to support patient needs and the Provider's plan of care through a team-based primary care approach.
- Complete data entry, patient documentation / record keeping, and reporting accurately and within required time frames according to program guidelines.
- Support of site community health services and educational events through planning, setting up, tearing down, and staffing events as needed .
- Ability to work with a diverse patient / family population, including both adult and pediatric populations.
- Ability to work independently, while collaborating with other team members both internally and externally of varying levels.
- Ability to interpret, adapt, and apply guidelines and protocols.
- Performs other related duties as assigned.
Qualifications
Excellent communication and interpersonal skills. Good oral, written and presentation skills. Bilingual (English / Spanish) strongly recommended .Proficient computer skills including Microsoft Office Suite, data entry, report retrieval and generation; experience documenting in Electronic Health Records and utilizing telehealth applications .High School Diploma or equivalent experience required .Medical Assistant Certificate highly preferred and may be substituted for similar clinical certifications, acumen or demonstrated experience.1-3 years' experience in healthcare required .2+ years of health coaching, wellness, health promotion, health outreach experience preferred. Health education or weight management background a plus.Must maintain a valid driver's license, vehicle, and appropriate insurance coverage.Ability to maintain a HIPAA compliant professional work environment.Operate with sense of urgency and comfort in a variety of environmental paces .Strong organizational skills and ability to work both independently and as part of a team.Physical Requirements
Must have reliable transportation and willing to travel within DFW area.Must be able to stand for various lengths of time.Must have stamina to perform manual duties that require strength, such as bending, standing, reaching, and lifting up to 30 lbs.Must be able to tolerate heat / outdoor working atmosphere.
