Job Description : \n\n \n\t

  • Patient Care :  Provide high-quality nursing care to patients in their homes, including administering medications, wound care, and monitoring vital signs.\n\t
  • Care Coordination :  Collaborate with physicians, registered nurses, and other healthcare professionals to develop and implement individualized patient care plans.\n\t
  • Patient Education :  Educate patients and their families on disease management, home care procedures, and health maintenance.\n\t
  • Documentation :  Maintain accurate and detailed patient records, documenting care provided and patient responses.\n\t
  • Home Health Visits :  Conduct regular home health visits, ensuring a safe and comfortable environment for patients.\n\t
  • Compliance :  Adhere to all health and safety regulations, as well as company policies and procedures.\n

    Education and Certification Requirements :

    \n\n \n\t

  • Valid New Mexico LPN license.\n\t
  • Home health / hospice experience required.\n\t
  • Strong clinical skills and knowledge of home health care practices.\n\t
  • Excellent communication and interpersonal skills.\n\t
  • Ability to work independently and as part of a multidisciplinary team.\n
