Care Coordinator – Full Time

About the Role
The Care Coordinator plays a vital role in supporting an integrated, whole-person approach to health and wellness for individuals served at the MARC Healing Center. This position provides case management, treatment planning, and care coordination services to ensure that clients receive the resources and support they need during and after their time in the triage program.
The Care Coordinator works collaboratively as part of a multidisciplinary treatment team, maintaining close coordination with clinical, psychiatric, and community partners to promote recovery and continuity of care.
Key Responsibilities
Evaluate treatment plans and goals with clients and monitor progress throughout care.
Coordinate care and collaborate with outpatient psychiatric and counseling services, emergency services, and other team members.
Connect clients with appropriate community resources and supports for after discharge from triage.
Make referrals for auxiliary services such as substance use treatment when clinically indicated.
Coordinate care with primary care and other professionals, as well as natural supports, to ensure holistic client care.
Conduct follow-up phone calls to check on client progress post-discharge.
Attend community-based and multi-system meetings as needed for care coordination.
Maintain accurate and timely documentation in accordance with agency and regulatory standards.
Meet or exceed established key performance and response time expectations.
Participate in supervision and team meetings.
Maintain compliance with all required trainings and certifications.
Uphold ethical standards and confidentiality in accordance with Compass Health’s HIPAA and policy requirements.
Demonstrate respect for diversity, cultural sensitivity, and trauma-informed care.

What You Bring
BA / BS / BSW in a Behavioral Science-related field.
Experience in care coordination or case management preferred.
Knowledge or interest in Evidence-Based Practices.
Must submit an Agency Affiliated Counselor application upon hire if not already licensed or associate licensed through WA DOH.
Current First Aid & CPR certification (or ability to obtain within 90 days).
Valid WA State Driver’s License, vehicle, and insurance.
Bilingual / bicultural candidates encouraged to apply.
Skills & Abilities
Trained or willing to be trained in Non-Violent Crisis Intervention.
Ability to work effectively with individuals with complex and multi-system needs.
Positive, solution-focused demeanor when addressing challenges.
Strong organizational and multitasking abilities in a fast-paced environment.
Excellent written and verbal communication skills.
General computer proficiency, including familiarity with electronic medical record (EMR) systems.
Ability to pass pre-employment background check.

Physical Demands / Work Environment
The position is based in a clinical office environment. The employee must occasionally lift and / or move up to 20 pounds and is regularly required to sit, stand, walk, talk, and use close vision. The noise level in the environment is usually moderate.
