Job description :
- Develop and implement the nutrition services program in collaboration with the interdisciplinary team (IDT).
- Conduct nutrition assessments to determine the needs, preferences, and goals of participants.
- Participate in IDT meetings to develop participant care plans.
- Deliver and document direct care nutrition interventions in accordance with physician orders and care plans.
- Implement and monitor special diets and provide nutrition counseling to participants and families.
- Oversee menu planning and meal delivery programs to ensure safety and regulatory compliance.
- Direct nutrition staff, including hiring, training, overseeing, and mentoring.
- Guide the intake and enrollment process for prospective participants.
Qualifications :
Registered Dietitian with at least four (4) years of experience directing nutrition services in a similar setting.Current Basic Life Support (BLS) / First Aid Certification required.Reliable means of transportation.Excellent leadership, organization, and communication skills in settings with seniors, their families, and IDT members.At least one (1) year of experience with frail or elderly populations.Why is This a Great Opportunity :
