This Intake Coordinator role is an exceptional opportunity to provide excellent service to the local community working with a diverse population each day. In this position, your daily work has an immediate and positive impact on others. Some of the responsibilities include :

• Receive and document referrals
• Conduct preliminary screening and verify Medi-Cal eligibility
• Providing referrals to other services when needed
• Serve as the primary point of contact for intake-related inquiries
• Daily interaction with the team to provide excellent communication. Minimum requirements are :
• Associate’s or Bachelor’s degree in social work, healthcare administration, public health, or related field.
• Experience with managed care programs.
• Knowledge of community-based service coordination.
• Experience providing client and / or family education regarding public program benefits.
• Demonstrated ability to collaborate with multi-disciplinary teams to streamline authorization and service coordination processes.

Hours are M-F 8AM-5PM and we are hiring immediately, please apply today.
