Home Health Physical Therapist

Join our client's dedicated Home Health team as a Physical Therapist , where you'll make a meaningful difference in the lives of patients by providing personalized care in the comfort of their homes. Key Responsibilities

• Conduct comprehensive patient assessments and create individualized physical therapy care plans
• Implement therapeutic interventions to improve patient mobility, strength, and overall function
• Educate patients and caregivers on home exercise programs, equipment use, and safety techniques
• Supervise Physical Therapy Assistants and other clinical support staff as needed
• Maintain detailed and timely clinical documentation using EMR systems
• Collaborate with interdisciplinary healthcare teams to ensure coordinated and effective patient care
