General Statement of Duties
Responsible for providing comprehensive primary care services in a primary medical care, ambulatory clinic setting. The provider works independently and interdependently with other clinic providers.
Responsibilities and Essential Functions
- Provision of comprehensive primary care services for all ages in a primary medical care ambulatory clinic setting. Works independently and interdependently with other medical providers and visiting specialists.
- Provide care consistent with evidence-based medicine.
- Perform minor outpatient procedures in accordance with the scope of practice and privileging policy.
- Provide consultation to other clinic providers either in person or by telephone.
- Consult with clinic physicians, specialists, or other health care professionals as appropriate.
- Assess patient needs for specialty referrals, diagnostic and in-patient care and make appropriate referrals.
- Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit. Verify and document that the patient understands diagnosis, treatment and follow-up recommendations.
- Participate in a shared call system with other providers.
- Assist with meeting clinic care team productivity and quality care standards.
- Participate in audits, and meet standards of quality Improvement, risk management and productivity goals.
- Complete accurate and timely documentation in patient record within 24-48 hours after patient visit.
- Maintain credentials essential for practice, to include licensure, certification and CME.
- Demonstrates respectful, professional and appropriate behavior that supports a team-oriented work environment.
- Attend and participate in provider meetings, in-services, and agency wide meetings.
- Demonstrate a commitment to the mission, core values and goals of LCHC and its healthcare delivery practices.
- Perform other duties as assigned.

Intermediate or advanced knowledge of Microsoft Office Products : Excel, Outlook, Word, and Power PointKnowledge of Federally Qualified Health CentersMinimum of two years as licensed Physician AssistantPhysical Demands Required to Fulfill Essential Functions of this Position
Employee must be able to : sit or stand for long periods of time; focus on tasks while in an active office environment where conversation and noise is prevalent; operate a keyboard, write, speak, and hear; read small print both on paper and on a computer screen for long periods of time and, occasionally lift up to 20 pounds.
