Description :
Accountable for preparing, organizing, and assisting Case Managers.
Coordinate and maintain all office management functions.
Independently manage patient discharge functions such as transportation and nursing home returns.
Intervene with insurance companies on utilization review activity.
Conduct monthly calls to all assigned members and ensure compliance with HIPAA verification.
Utilize monthly assessments and other social determinants of health assessments when speaking with members.
Employ critical thinking and judgment when dealing with unplanned issues.
Escalate clinical issues to designated care managers.
Accurately and timely document member call interactions in the care management system and other applicable software programs.
Ensure compliance with professional standards and regulatory guidelines.
Ensure members have appointments with their providers.
Fulfill basic care coordination tasks for members, such as scheduling appointments and addressing transportation and medication issues.
Conduct other supportive activities as assigned.
Problem-solve member issues in an independent and resourceful manner.
Perform non-clinical activities for members of varying age, clinical scenarios, culture, financial means, social support, and motivation.
Engage members and caregivers in a collaborative relationship, empowering them to manage their physical, psychosocial, and environmental health.
Promote communication to enhance the effectiveness of clinical and non-clinical services and overall case management services.
Maintain accurate and complete documentation of required information in compliance with risk management, regulatory, and accreditation requirements.
Protect the confidentiality of member information and adhere to all company policies regarding confidentiality.
Collect and prepare data, statistics, proposals, and reports relevant to responsibilities as a care coordinator.
Provide care coordination that reduces avoidable utilization, increases member satisfaction and retention, and reduces gaps in care.
