About The Position

Job Title : Occupational Therapist

Profession : Rehabilitation Therapy

Shift : Monday to Friday, Days

Hours per Shift : 32 guaranteed hours

Experience : New graduates are welcome. SNF experience preferred.

License : Valid occupational therapy license required.

Certifications : COVID-19 test required within 72 hours prior to start date with results issued 48 hours prior.

Must-Have : Completed prescreen form and attached resume and license verification.

Description :

Candidates must ensure the prescreen form is filled out and attached upon submission.

If the prescreen form, resume, and license verification are not completed and attached, the candidate will be declined.

Three references are required.

A county criminal background check (7 years for all counties lived and worked) is necessary.

Federal criminal background check (7 years or more) is required.

A multi-state criminal search (7 years or more) must be conducted.

A nationwide sex offender search (7 years or more) is mandatory.

HHS / OIG and GSA searches are essential.

A social security number trace must be completed.

A 1-step PPD or Quantiferon test is required or proof of positive PPD and chest X-ray.

A SAM search is necessary.

Negative drug screen results are required.

A physical examination is mandatory.

A Fire Safety Card from LA must be obtained every 4 years.

Position urgency is normal.

Holiday coverage is not required.
