About the Position :

  • Inpatient / Outpatient Hybrid Structure : 80% outpatient and 20% inpatient care.
  • Patient Load : Approximately 15-20 outpatient and 5 inpatient patients per day.
  • Appointment Structure : Patients scheduled in 20-40-60-minute intervals depending on appointment type for personalized care.
  • APP Supervision : Oversight and collaboration with advanced practice providers.
  • Embedded Care Coordinator : Enhancing patient care coordination and support within the practice.
