Description
Provide assistance with light housekeeping needs that are directly related to the health and well-being of the eligible individual, i.e., bed-making, dusting, vacuuming, etc.
Provide assistance with accessing community resources / shopping / appointments.
Accurately complete and submit Daily Activity Logs and Contact Summaries in a timely manner.
Adhere to safe transportation procedures.
Follow appropriate protocol for completion of I / A and Serious Incident reports.
Ability to work a flexible schedule based on the family’s needs.
Attend trainings / in-services as requested and adhere to requests for personnel update information in a timely manner.
Adhere to Activity Plan and / or family instructions regarding personal care of the eligible individual.
Keep supervisor abreast of pertinent information relevant to the provision of services.
Follow all safety policies and adhere to all worker's compensation program guidelines
OTHER FUNCTIONS :
Perform other functions and tasks as assigned.
EDUCATION / EXPERIENCE / SKILL REQUIREMENTS :
High School Diploma or equivalent required. Must be at least 21 years of age.
LICENSES / DESIGNATIONS / CERTIFICATIONS :
Must have a valid driver's license.
