Nurse Practitioner Job Description

Working collaboratively as a member of the medical sta, the APRN will play a critical role in managing the medical needs of skilled nursing facility residents. The APRN will establish and build strong working relationships with administrators and sta throughout their designated building and will act as the primary point of contact for emergencies and urgent care visits. Ultra Health provides our APRNs with the training and tools to do more of what you love working independently to see patients and treat complex medical conditions. Ultra Health oers a highly competitive base salary, plus performance-based bonus.

Nurse Practitioner Responsibilities

Provide primary, urgent and preventative care for residents in assigned skilled nursing facility. May also provide care to assisted living facility residents.

Verify admission orders and conduct initial assessment of residents. Conduct Physical exams, medication review, and depression screening

Collaborate with facility medical director, attending physicians and nursing sta to deliver the highest level of care to the patient

Communicate with and counsel family members on treatment options when there is material changes in status

Minimum Qualications

Valid Nurse Practitioner License in Indiana

DEA License

Nurse Practitioner national certication

A clinical background in adult, family or geriatric specialties

Vaccination against COVID-19 is required for all new hires unless the individual qualifies for a remote, religious, or medical exemption.

Preferred Qualications

Experience in internal medicine, gerontology, or cardiology

Excellent administrative, organizational and verbal skills

Ability to work independently

Detail oriented

Dependable and reliable

Nursing experience in LTC / SNF a plus

$100,000 - $150,000 a year

Compensation details : 100000-150000 Yearly Salary
