We are actively hiring Physical Therapists (PTs) for home care opportunities in :

Westchester County, NY

Rockland County, NY

Bronx County, NY

Key Responsibilities

Assessment : Evaluate patients' functional limitations and develop personalized therapy plans with patients, families, nurses, and physicians.

Direct Care : Deliver therapy services using accepted physical therapy modalities for medical and surgical conditions.

Education : Train and supervise patients, families, and home health aides on exercise programs and care techniques.

Ongoing Evaluation : Monitor and adjust therapy plans as patients' conditions evolve.

Communication & Documentation : Report updates to physicians and managers, and maintain accurate clinical records of evaluations, treatments, and outcomes.

Qualifications

Active New York State Physical Therapist (PT) license & registration (required)

Minimum 1 year of full-time physical therapy experience

Home care experience preferred (not required)

Ability to use technology (tablets) for documentation; Oasis experience a plus

Valid driver's license and reliable automobile required for field visits

Compensation & Benefits

Salary : $95,000 - $108,000 annually (based on experience)
