Registered Nurse Field Case Manager - Part Time

• Referral program\nResponsibilities of an RN Case Manager : \n
• No OASIS work!\n
• Provides care as defined within the scope of the states nurse practice act.\n
• Makes the initial evaluation visit and regularly reevaluates the clients nursing needs.\n
• Prepares clinical and progress notes and summaries of care.\n
• Serve as backup for Critical Care Clients, In-School Staffing & PDN\n
• Assures proper maintenance of clinical records in compliance with local, state, and federal laws.\n
• Helps educate and coach care-giving staff and assists with new hire clinical training, as needed.\nRN Case Manager Requirements : \n
• Current unrestricted RN license in Minnesota\n
• Minimum two (2) years of clinical experience with one (1) year of experience in a home care agency, primary care clinic, or health facility, preferred.\n
• Critical Care Experience (1-2 years)\n
• Active CPR Certification\n
• Negative TB skin test or QuantiFERON-TB Gold (QFT) blood test (completed within the last 90 days)\n
• Reliable car & Auto Insurance\n
• Valid Driver's License\n
• Clean background and Criminal Record\n
• Understands regulatory compliance with state and federal regulations.\n
