Shift : Weekday daytime schedule, with rotating weekends and on call as needed

Job Description :

Sunbelt Staffing is hiring an experienced Home Health RN to join a leading home health agency in Lexington, SC. This role requires strong clinical skills, the ability to manage a caseload efficiently, and experience with OASIS and Homecare Homebase documentation. Nurses in this role will provide essential care in patients homes while collaborating closely with an interdisciplinary team.

Key Responsibilities :

Perform skilled nursing visits and OASIS assessments

Provide patient-centered care and medication management

Educate patients and caregivers on treatment and wellness practices

Maintain accurate documentation in Homecare Homebase (HCHB)

Coordinate with physicians, therapists, and care staff to ensure quality outcomes

Participate in weekend rotation and on call as scheduled

Qualifications :

Active RN license in South Carolina or compact state

At least 2 years of nursing experience, with a minimum of 1 year in home health

Required : Proficiency in OASIS and Homecare Homebase

BLS certification

Reliable transportation, valid drivers license, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
