This role focuses on start-of-care visits and patient reassessments for both skilled and non-skilled clients in their homes.\n\nThis is an ideal opportunity for an RN who values flexibility, autonomy, and the ability to make a meaningful impact on patients' lives.\n\nKey Responsibilities : \n

• Conduct start-of-care assessments for new clients, evaluating medical, functional, and psychosocial needs\n
• Perform reassessments for ongoing clients to ensure care plans remain appropriate and effective\n
• Identify skilled and non-skilled needs, collaborating with the care team to develop and update individualized care plans\n
• Educate clients and families on care strategies, safety, and wellness goals\n
• Document all assessments and interventions accurately in the agency's EMR system\n
• Communicate findings and recommendations to the interdisciplinary care team\n
• Adhere to all regulatory, agency, and clinical policies\n\nQualifications : \n
• Current Registered Nurse (RN) license in VA\n
• Minimum 1 year of nursing experience; home care experience preferred\n
• Strong clinical assessment skills and critical thinking\n
• Knowledge of both skilled and non-skilled home care requirements\n
• Reliable transportation and valid driver's license\n
• CPR certification\n
