Summary
The RN Supervisor- Clinic and Care Coordination is responsible for providing day-to-day clinical leadership and
operational oversight of care coordination services (Chronic Care Management, Transitional Care Management, and
general case management), as well as overall clinic nursing operations. This position works in partnership with clinic
management, providers, and nursing staff to ensure high-quality, patient-centered care, optimize clinical workflows,
ensure regulatory compliance, and support care transitions and chronic disease management.
This role is vital to ensuring staff are competent, confident, and operating within their scope of practice, while also
maintaining a strong presence in provider relations, patient experience, and care continuity. The RN Supervisor
functions as both a strategic and hands-on leader, balancing administrative responsibilities with direct patient care and
mentorship when needed.

Clinical Oversight & Supervision
Rotate between all clinic locations on a regular scheduleProvide daily clinical supervision and leadership to RNs, LPNs, Clinical Assistants (MAs), and care coordinationstaff.
Delegate and manage daily nursing assignments based on staff scope of practice, patient care needs, andcoverage demands.
Supervise care coordination programs including CCM and TCM, ensuring accurate documentation, patienteligibility tracking, time logging, and billing workflows.
Serve as a clinical escalation resource for care coordination and complex patient cases.Monitor patient care delivery, promoting evidence-based practices and quality improvement.Staff Development & Performance Management
Participate in recruitment, onboarding, performance evaluations, staff development, and corrective actions.Identify training needs and facilitate or coordinate regular in-services and competency validation.Ensure staff maintain current credentials and complete mandatory trainings.Provide hands-on training and mentorship for both new and existing staff in lab, clinic, and care coordinationworkflows.
Regulatory Compliance & Quality Assurance
Ensure compliance with CMS, MDH, CDC, OSHA, and other regulatory bodies related to clinical operations anddocumentation.
Oversee clinic infection prevention practices and proper storage / handling of medical supplies and vaccines.Conduct internal audits and mock surveys to ensure clinical readiness and documentation quality.Assist in development, implementation, and revision of policies, procedures, and workflows.Leadership & Program Implementation
Serve as a liaison between clinical support staff and medical providers to address issues, improvecommunication, and enhance collaboration.
Lead or participate in strategic initiatives and clinical committees (e.g., Pain Committee, Diabetes AdvisoryCommittee).
Support implementation and evaluation of new care models, including the programs like chronic caremanagement.
Assist in planning and resource allocation for new programs or population health initiatives.Patient Care & Community Engagement
Provide direct patient care when necessary, especially during staffing shortages or peak patient volume.Support community health efforts, including wellness initiatives, chronic disease management, and preventionprograms.
Engage in patient education, especially for chronic conditions such as diabetes, hypertension, COPD, and caretransitions.

Required Education and Experience
Current, unencumbered Registered Nurse (RN) license in the State of Minnesota.BLS certification required.Bachelor of Science in Nursing (BSN) preferred;Minimum 3 years of RN experience in ambulatory care, with at least 1 year in a supervisory, care coordination,or case management role.

Provide leadership and guidance to the department Monday through Friday from 8 : 30 a.m. to 5 : 00 p.m., with rotating
Saturday mornings. Hours may vary earlier or later depending on workload. This role will rotate through clinic locations
on a set schedule, working in different clinics as assigned. Additionally, availability outside of regular business hours may
occasionally be required to address urgent questions or operational needs.
Supervisory Responsibility
This position directly supervises RN / LPN care coordinators, clinical assistants (MAs), and other clinical support staff
across family medicine clinics.
