Your Role

As a vital member of our mobile wound care team, you will :

  • Provide hands-on, bedside wound care to patients in skilled nursing and assisted living facilities.
  • Lead wound assessments, treatment planning, and advanced procedures.
  • Utilize cutting-edge technology to enhance healing processes.
  • Make informed decisions based on a holistic approach to patient care.
  • Maintain accurate and timely charting in our customized EMR system.
  • Recommend next steps and refer patients as needed for optimal care.
  • Collaborate with NPs / PAs, providing real-time support and shaping clinical standards.
  • Drive quality initiatives and support our mission to elevate post-acute care.

    • Qualifications

  • MD or DO with an active and unrestricted Illinois license.
  • Board Certification in Internal Medicine, Family Medicine, Surgery, or a related field.
  • At least 2 years of physician experience (wound care or post-acute preferred).
  • Illinois prescriptive authority & DEA registrations.
  • Willingness to serve as a collaborating physician.
  • Ability to travel daily to assigned facilities in the Chicago area.
  • Comfortable with EMR and mobile documentation.
  • Strong interpersonal skills—collaborative, communicative, and adaptable.
  • Committed to hands-on patient care and mentoring advanced practice providers.
