Job Overview

The Program Manager for Street Medicine provides direct leadership and daily oversight for AltaMed's Street Medicine program for clinically and socially complex unhoused patients residing in AltaMed's communities. The Manager is responsible for the successful development and execution of the street medicine program within AltaMed's Primary Care Model. The Manager collaborates with all stakeholders and cross-functional departments, including health education, enhanced care management, behavioral health, senior and complex care, and AltaMed Health Network. In addition, the Manager develops and executes operational goals, objectives, initiatives, and action plans to achieve outcomes by identifying and monitoring key service line targets and metrics. The Program Manager is accountable for monitoring and improving Street Medicine operational processes, along with training and performance management of the Street Medicine program to ensure all services are meeting or exceeding critical performance measures in grant and health plan compliance, strategic goals, and mission and vision of the corporation. Lastly, the Manager drives and participates in policy and community advocacy work with internal and external entities to advance legislative changes affecting unhoused populations and maintains and expands collaborative efforts with multiple community agencies providing these services.

Minimum Requirements

  • Bachelor's Degree in Business, Health Administration, Nursing (RN), or other health-related field required.
  • Master's Degree in Business, Public Health, Nursing (RN), Social Work, or Health Administration preferred.
  • Minimum of 3 years in a leadership or management role with experience in health care administration or a closely related field required.
  • Experience in financial management, grant management, program development, personnel management, project management, and quality management required.
  • Experience working with government and private sector entities, and prior experience managing contracts.
  • Experience working with unhoused populations preferred.

Compensation

$100,692.80 - $151,465.60 annually

Actual salary offers are considered by various factors, including budget, experience, skills, education, licensure and certifications, and other business considerations. The range is subject to change. AltaMed is committed to ensuring a fair and competitive compensation package that reflects the candidate's value and the role's strategic importance within the organization. This role may also qualify for discretionary bonuses or incentives.
