Summary of Job Duties : We are seeking dynamic and dedicated Community Health Workers (CHWs) to join our Postpartum Call Center team. This role offers a unique opportunity to make a meaningful impact on maternal health outcomes in Arkansas through education, outreach, and community engagement. CHWs will collaborate with patients, community-based organizations, and local stakeholders to provide education, resources, and health support. Key responsibilities include individual goal planning, case management, group education, and culturally appropriate outreach for hard-to-reach populations.
