As a Medication History Pharmacy Technician, you will play a crucial role in ensuring high-quality patient care by accurately compiling medication histories for patients upon admission.

Key Responsibilities :

  • Conduct interviews with patients and family members to gather an accurate list of medications prior to hospital admission.
  • Contact community pharmacies or medical offices for clarification when necessary.
  • Document medication histories and update electronic health records with complete accuracy.
  • Communicate any unresolved issues regarding medication history to pharmacy specialists.
  • Assist in preparing educational materials for patient discharge counseling.
  • Maintain compliance with ongoing education and certification requirements.
  • Participate in regular staff meetings and collaborate with team members.
  • Qualifications :

  • High school diploma or GED is required.
  • 2 years of retail pharmacy experience is mandatory; inpatient pharmacy experience is preferred.
  • Active pharmacy technician or pharmacy intern license is required.
  • Board Certification as a Pharmacy Technician is preferred.
