Job Description :

We are seeking a compassionate and experienced Long Term Care / Skilled Nursing Facility (LTC / SNF) Registered Nurse (RN) to provide high-quality care to residents. This role focuses on the long-term health and well-being of residents, managing chronic conditions, and providing skilled nursing care. The ideal candidate has strong clinical assessment skills, is a patient and effective communicator, and is passionate about building relationships with residents and their families.

Key Responsibilities :

Provide direct nursing care to residents with a wide range of medical needs.

Administer medications and treatments according to care plans.

Monitor resident health status and report any changes to the healthcare team.

Collaborate with physicians, therapists, and other staff to develop and implement individualized care plans.

Educate residents and families on health management and disease prevention.

Ensure accurate and thorough documentation of all care provided.

Qualifications :

Active RN license in Minnesota or a compact license.

Associate’s or Bachelor’s Degree in Nursing.

Basic Life Support (BLS) certification.

Minimum of 1 years of recent LTC / SNF nursing experience.

COVID vaccine and recent TB test may be required.
