Are you ready to make a difference in patient care? As a Hospitalist Nurse, you will play a vital role in collaborating with our dedicated hospital care team and floor staff. Your mission? To ensure seamless patient care from admission through discharge, all while providing exceptional support to our patients and their families.

What You'll Do

In this dynamic role, you will :

• Optimize Patient Care : Initiate and assist with department operations to enhance patient experiences.
• Assess and Prepare : Evaluate patients' clinical status and gather essential information for physician assessments.
• Lead with Communication : Provide leadership through effective communication and validation of patient care.
• Enhance Workflow : Proactively contact various departments to improve efficiency.
• Facilitate Discharges : Conduct afternoon meetings with the Hospitalist team to discuss potential next-day discharges.
• Improve Quality of Care : Collaborate with Hospitalists to enhance quality improvement efforts.
• Prevent Readmissions : Identify preventable readmissions and develop targeted care plans.
• Follow Up : Connect with patients post-discharge to update them on new results and care plans.
• Maintain Records : Keep the Epic in-basket organized and efficient.
• Educate : Provide valuable education to patients, families, and fellow RNs regarding patient needs.

Compensation

The compensation range for this position is $32.81 - $56.43 per hour . This range reflects our commitment to fair compensation based on experience, qualifications, and other factors.
